Upgrading to Class 2 reptile keepers licence

Hi Guys

I am new to this site...first posting, very exciting!

Just wondering if anyone knows where I can find the NSW form to upgrade from Class 1 licence to Class 2...can only seem to find the original licence application form on the NPWS website.

I know there is certain criteria that is required to be met to upgrade to the class 2, I do meet these conditions...so know all that is left to find the form and pay the additional money. My new snake is waiting for me...
