my coastal has not been eating his pinky's head first. can this cause problems?

Not a problem at all, it's fine!

Pinky mice or rats? Swallowing backwards wont hurt him.

If you think he's big enough, try a pinky rat. Make sure you present it head first and that he grabs the head. I find this usually works.
