Licensing in WA

Umm...after finally mustering up the guts to just get a snake and to hell with the bf....I've gone and lost the application forms! I can't remember where I downloaded them from and I forgot to save an e-copy. I've tried googling and going to the DEC website but everything on google is for nsw and qld and the DEC website is bugged so none of the links lead anywhere. Can someone please help me find the forms or send me a copy?
